3/26/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. She verbalized to QMHPA that her morning was going okay. QMHPA informed Ms. Broadnax that this session was going to focus on addressing and maintaining healthy relationships. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings and emotions. QMHPA assisted Ms. Broadnax with applying proper calming techniques when feeling depressed. QMHPA explained to Ms. Broadnax walking, listening to music, and/or journaling are good coping mechanisms. QMHPA processed with Ms. Broadnax identifying enjoyable activities to assist with coping with feelings of anger and depression. QMHPA addressed progress that Ms. Broadnax has made towards building positive relationship with others. QMHPA displayed positive communication skills throughout the session. QMHPA challenged Ms. Broadnax to use positive communication when interacting with others. QMHPA praised Ms. Broadnax for her active participation during the session. Ms. Broadnax appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. Ms. Broadnax informed the QMHPA that she does not try to implement proper calming techniques. Ms. Broadnax stated âI can be honest that I do not do well with applying any of the coping skills that you have taught me. I rather sometimes just handle situations like I am used to. I know it is not the best thing for me to do but I do not always feel that the people that I am upset with deserve me being nice to themâ. Ms. Broadnax conveyed that she likes watching television, and spending time with her boyfriend. Ms. Broadnax noted that she has stopped trying to build a healthy relationship with her son because they cause her too much stress. Ms. Broadnax noted that she continues to stay in the house so that she does not have to interact with any of the individuals in the home that trigger feelings of anger and depression.
